This document describes certain limitations and exclusions of liability applicable to the use of the My Medical Getaway Inc. / Mon Séjour Médical Inc. website and to the administrative and logistical coordination services offered to clients.

My Medical Getaway Inc. / Mon Séjour Médical Inc. acts only as an administrative and logistical coordination service for planned medical stays abroad.

2. Role of My Medical Getaway Inc. / Mon Séjour Médical Inc. — Coordination Only

My Medical Getaway Inc. / Mon Séjour Médical Inc. acts only as an administrative and logistical coordination service.

My Medical Getaway Inc. / Mon Séjour Médical Inc. does not provide medical care, make diagnoses, prescribe treatments, provide medical advice, provide clinical recommendations, select providers based on a medical evaluation or replace the assessment of a qualified healthcare professional.

Any clinical decision, medical recommendation, assessment, treatment, procedure, final medical price, medical timeline, availability, complication or outcome remains the sole responsibility of the independent healthcare professionals, clinics, hospitals or other medical providers concerned.

3. Responsibility of Independent Providers

Medical services, including consultations, examinations, procedures, prescriptions, follow-ups, medications, clinical decisions or other healthcare services, are provided exclusively by independent medical providers.

Non-medical services, including transportation, accommodation, excursions, insurance, agencies, interpretation or other services related to the stay, where applicable, are provided by independent third parties according to their own terms, policies, availability, pricing, insurance coverage, limitations and responsibilities.

Each independent provider acts in its own name and under its own responsibility. My Medical Getaway Inc. / Mon Séjour Médical Inc. does not control the acts, omissions, decisions, practices, timelines, availability, pricing, policies, complications or outcomes of these providers.

4. Local Transportation and Optional Services

When local transportation is requested, My Medical Getaway Inc. / Mon Séjour Médical Inc. acts only as an administrative coordinator between the client and an independent transportation provider, where available.

When the arrival transfer between the airport and the accommodation is included in a coordination package, it remains provided by an independent transportation provider, subject to availability, schedules, terms, insurance coverage, policies and applicable limits. Other local transfers may be coordinated as an optional service, where available.

My Medical Getaway Inc. / Mon Séjour Médical Inc. does not act as a carrier, operator, mandatary, agent or contracting party for the transportation service provided by a third party.

The independent transportation provider remains responsible for the performance of the transportation service, including its vehicles, drivers, schedules, insurance coverage, terms, delays, cancellations, incidents, damages or changes.

To the extent permitted by applicable law, My Medical Getaway Inc. / Mon Séjour Médical Inc. shall not be liable for losses, damages, delays, incidents, cancellations or disputes related to transportation services provided by an independent third party.

5. Referrals, Introductions and Other Options

Referrals, links, contact information or introductions to third-party providers, including agencies, accommodation providers, insurers, transportation providers, excursions, interpreters or other suppliers, are optional and offered upon the client’s request, where available.

My Medical Getaway Inc. / Mon Séjour Médical Inc. may facilitate an introduction or administrative coordination, but does not provide these third-party services.

Any booking, confirmation, payment, change, cancellation or refund remains exclusively governed by the relevant third-party provider and its own terms.

6. Final Choice of Independent Provider by the Client

My Medical Getaway Inc. / Mon Séjour Médical Inc. may present or facilitate introductions to certain facilities, clinics, physicians, healthcare professionals or independent providers, depending on the information provided by the client, the destination considered, communicated availability and the general nature of the request.

The final choice of provider belongs to the client, who remains free to accept, refuse or consult any other provider of their choice.

My Medical Getaway Inc. / Mon Séjour Médical Inc. does not provide medical recommendations, does not select a provider based on a clinical evaluation, and does not guarantee the quality, outcome, availability, timelines, final medical pricing or medical result of services provided by an independent provider.

7. No Guarantee — Timelines, Availability, Pricing and Outcomes

My Medical Getaway Inc. / Mon Séjour Médical Inc. does not guarantee:

• acceptance of a file by an independent provider;

• availability of a physician, clinic, hospital or other provider;

• response time or appointment timelines;

• the final or definitive accuracy of an estimate before assessment by the provider concerned;

• a final medical price or the absence of additional fees;

• a clinical, medical or aesthetic outcome;

• absence of complications;

• reimbursement by insurance;

• availability of transportation, accommodation, interpretation or third-party services;

• continuity, safety, punctuality or performance of a service provided by an independent third party;

• authorization to enter a foreign country.

Timelines, availability, final medical pricing, recommendations, clinical decisions, complications or outcomes depend, among other factors, on independent providers, file completeness, medical assessment, selected destination, required documents and circumstances beyond the control of My Medical Getaway Inc. / Mon Séjour Médical Inc.

8. Accuracy of Information Provided by the Client

The client is responsible for the accuracy, completeness and updating of the information provided to My Medical Getaway Inc. / Mon Séjour Médical Inc., including health-related information, documents, availability, contact information and travel information.

My Medical Getaway Inc. / Mon Séjour Médical Inc. may suspend, delay or limit coordination if the information provided is insufficient, inconsistent, incomplete or not updated.

To the extent permitted by applicable law, My Medical Getaway Inc. / Mon Séjour Médical Inc. shall not be liable for consequences resulting from inaccurate, incomplete or outdated information provided by the client.

9. Emergencies

My Medical Getaway Inc. / Mon Séjour Médical Inc. does not provide emergency medical services.

In the event of a medical emergency, complication, concerning symptom or situation requiring immediate assistance, the client must immediately contact local emergency services, the medical facility concerned or a qualified healthcare professional.

My Medical Getaway Inc. / Mon Séjour Médical Inc. does not replace emergency services, physicians, hospitals, insurers or consular services.

10. Separation of Payment Flows

My Medical Getaway Inc. / Mon Séjour Médical Inc. charges only its administrative and logistical coordination fees.

Medical fees, including consultations, examinations, procedures, treatments, medications, hospital fees, professional fees or other medical services, are established, billed and paid directly to the independent medical providers concerned.

Travel, optional local transportation, accommodation, insurance, excursion or other non-medical service fees are established, billed and paid according to the policies of the independent providers concerned, unless otherwise clearly provided in the coordination package.

My Medical Getaway Inc. / Mon Séjour Médical Inc. is not responsible for the payment, cancellation, refund, pricing, availability, terms or service delivery policies of independent providers.

11. Limitation of Liability

To the extent permitted by applicable law, My Medical Getaway Inc. / Mon Séjour Médical Inc. shall not be liable for damages, losses, costs, delays, cancellations, refusals, decisions, complications or consequences arising, in particular, from:

• the acts, omissions, decisions, pricing, timelines, policies, complications, outcomes or services of independent medical or non-medical providers;

• a refusal, postponement, change, cancellation or modification of an appointment by an independent provider;

• a change in price, estimate, availability, planning, treatment or terms by an independent provider;

• a delay, cancellation or change in flights, transportation, accommodation or activities;

• refusal of entry, visa issues, border requirements or an administrative decision by an authority;

• incomplete, inaccurate or late information provided by the client or by an independent provider;

• medical complications, unsatisfactory results or an unexpected change in the client’s health condition;

• any issue related to insurance, coverage, exclusions or reimbursement.

Nothing in this document is intended to exclude or limit any liability that cannot be excluded or limited under applicable law.

12. Informational Content — No Medical Substitution

Any content provided by My Medical Getaway Inc. / Mon Séjour Médical Inc., including website texts, guides, itineraries, informational documents, journey explanations or general communications, is provided for informational, administrative and organizational purposes only.

My Medical Getaway Inc. / Mon Séjour Médical Inc. does not control these external websites or services and is not responsible for their content, operation, policies, privacy practices, interruptions or terms of use.

14. Application of Legally Permitted Limits

These limitations and exclusions apply to the extent permitted by applicable laws.

Nothing is intended to exclude or limit any liability that cannot be excluded or limited under applicable law.

15. U.S. Clients and HIPAA

If the client is located in the United States, the client understands that My Medical Getaway Inc. / Mon Séjour Médical Inc. is an administrative and logistical coordination company.

Unless otherwise expressly agreed in writing, My Medical Getaway Inc. / Mon Séjour Médical Inc. does not act as a covered entity or business associate within the meaning of the U.S. Health Insurance Portability and Accountability Act, commonly known as HIPAA.

Independent medical providers may be subject to their own legal, professional or regulatory obligations depending on their jurisdiction.

16. Governing Law and Jurisdiction

This document is governed by the applicable laws of the Province of Québec and the applicable federal laws of Canada.

Subject to mandatory consumer protection rights, any dispute relating to the services of My Medical Getaway Inc. / Mon Séjour Médical Inc. shall be submitted to the competent courts of the Province of Québec.
